Overview
Passports are issued by the Controller of Immigration and Emigration in Colombo and not by the Sri Lanka High Commission in Kuala Lumpur. Citizens of Sri Lanka with residence in Malaysia can apply for a passport at the High Commission. The application will be forwarded to the Immigration Department in Colombo where a machine-readable passport is issued. After receiving the passport from Colombo, the High Commission will inform the applicant to call over at the High Commission and collect the passport.

A Sri Lankan passport is now issued for a period of ten years. After 10 years Sri Lankans are expected to apply for new passports. New passports can also be obtained when there are no more pages for visa endorsements.

In the case of “L” and “M” series passports, names of children under 16 years of age will be endorsed in either of the parent’s passports. Since the inclusion of children’s photographs in “N” series passports has now been automated, the High Commission is unable to entertain requests for inclusion of child/children’s details into the parent’s passports.

However, the High Commission could issue a Non-Machine Readable Passport or an Emergency Certificate as a separate travel document until such time the Department of Immigration and Emigration in Colombo processes the application for a new “N” series passport.

 

Issuance of Visas

Social/Tourist Visas
::
Malaysian Nationals visiting Sri Lanka as tourists are permitted to enter Sri Lanka without prior visa and will be given
30 day visas on arrival.

::Those who wish to obtain prior visas may obtain visas by submitting valid passport and return air ticket.

Business/ other visas (eg:seminars I Scholarships I Training etc)

Malaysian / foreign nationals visiting Sri Lanka on business / purposes other than business have to obtain visas prior to
visit Sri Lanka


Application for such visas have to submitted to the Sri Lanka High Commission together with the following documents / relevant visas.

:: Duly completed visa application
   (Can be downloaded from this website)

:: Passport of the applicant (Validity at least 3 months)

:: Two Passport size colour photographs of the applicant taken recently

:: Letter from the local employer / agency giving details and the purpose of visit with specific dates of  travel

Extension of Validity of Passports
1. Attach "O" Form, duly completed and signed and forward together with your passport.
2. FEES: RM 40 for a period of 5 years validity (RM 8 for each year). Fees should be paid at the High Commission and a receipt must be obtained. Please do not send your fees in cash by post. This High Commission cannot be held responsible for any cash sent by post. NO CHEQUES ARE ACCEPTED.
3. Extension of validity of passports can be done on the same day, upon submission of the documents by the applicant.

 

Issuance of New Passports in lieu of lost passports
Applicants are expected to come to the High Commission personally, together with the following documents. APPLICATIONS FOR NEW PASSPORT IN LIEU OF LOST PASSPORT CANNOT BE ACCEPTED BY MAIL.
Applicants for new Sri Lankan passports are expected to submit their applications to the High Commission in forms listed
at A below, together with supporting documents at B.

A. Forms
(i) Application for a new Sri Lankan travel document (Form 'K' (IM35), duly completed with the applicant's signature at space provided in page 2. Applicants are advised to give their present address in Malaysia at cage 6 on page 1.

(Applicants are advised to carefully read instructions given on page 3 of Application Form before completing the form).
(ii) Annexure to Form 'K' (IM35 (b))
Applicants are advised to read the instructions given on the form. Verification photograph and applicant's signature could be attested by an Officer at the High Commission at the time of handing in the Application. Place your signature on both cages in the presence of the Officer at the High Commission.
3 colour photographs for passports should be of the size 3.5cm x 4.5cm without a white border.
(iii) 'D' Form must be completed in duplicate. (Please do not leave any blanks.) You can download the 'D'-Form here.
(iv) If endorsement of child/children is required (only for “L” and “M” series passports), applicants are expected to complete Form 'C' (IM35) as well.
This form is applicable for deletion of names of children as well. The application should also be signed by the spouse or the legal guardian of the child/children. Three (3) colour photographs of the size 3.0 cm x 3.5 cm without white border, of each child to be included in the passport, have to be provided with the application along with supporting documents as stipulated in Form ‘C’. The names of the child/children should be written on the reverse of all three photographs.
B. Supporting Documents
(i) The original birth certificate of applicant together with a photocopy.
(ii) If the passport was stolen or lost in Malaysia, a certificate from the Police Station to which the theft was reported.
(iii) Translation in English of the police report by a sworn translator, if the report is in Bahasa Malaysia.
The High Commission will notify applicants by telephone in due course once the passport is received from the Controller Immigration and Emigration.
C. Fees
A Fee of RM 1420 for the issue of a new passport.
Fees for passports can be paid at the High Commission in cash. (Please note to obtain an official receipt for all payments made by you.)
Please do not send your passport fees in cash by post. NO CHEQUES ARE ACCEPTED.

Issuance of New Passports in lieu of passports, which have run out of pages and/or validity has
expired

Applicants for the above category are expected to submit their applications to the High Commission in forms listed at A below, together with supporting documents listed at B.
A. Forms
1. Form 'K' (IM35)
Applicants are advised to carefully read instructions given on page 3 of the Application Form before completing the form.
Form 'K' duly completed with the applicant's signature at space provided in page 2. Applicants are advised to give their present address in Malaysia at cage 6 on page 1.
2. Annexure to Form 'K' (IM35 (b))
Applicants are advised to read the instructions given on the form. If applicants cannot call over at the High Commission personally due various constraints, the verification of photograph and applicant's signature should be attested by a Notary/Commissioner of Oath in the area of residence.


Applicants are kindly requested to place their signature in both cages provided for signature. Please note that 3 colour photographs for passports should be of the size 3.5cm x 4.5cm without a white border.

Form 'C' If endorsement of child/children is required (only for “L and “M series passports), applicants are expected to complete Form 'C' as well. This form is applicable for deletion of names of children as well. The application should also be signed by the spouse or the legal guardian of the child/children.
Three (3) colour photographs of the size 3.0 x 3.5 cm without white border, of each child to be included in the passport, have to be sent with the application along with supporting documents as stipulated in Form ‘C’. The names of the child/children should be written on the reverse of all three photographs.

B. Supporting Documents
1. Passport currently held by the applicant.
2. Original Birth Certificate of Applicant and National Identity Card if available together with a photocopy.
3. Photocopy of personal data pages of current passport (in the case of M series passports inside page of the back cover and in other cases pages 2, 3, 4, 5, 6, &7).
C. Fees

The current fee for a new passport is RM 515.

If the passport held by the applicant has not been renewed at the appropriate time, fees normally charged for
extension of passport (RM 8 per year) should also be paid in addition to the fee for a new passport. Applicants are
>advised to pay passport fees in cash and obtain an official receipt. Applicants are strongly advised not to send their passport fees in cash by post. The High Commission cannot accept responsibility for any loss of cash sent by post. Personal cheques cannot also be accepted for payment of passport fees.


 
Issuance of New Passports for Children
If separate passports are applied for children, application should be made in Form 'K' (IM35) and Annexure to Form 'K'(IM35(b)). In addition, applications should be accompanied by the following documents:
1. Original Sri Lanka Birth Certificate of the child or the Citizenship Certificate issued by the Secretary of the Ministry of Defence in Colombo under Section 5 (2) of the Citizenship Act, No. 18 of 1948. Children born outside Sri Lanka can be issued separate passports only if Citizenship Certificates have been obtained.
2. Travel documents in which the names have been currently endorsed (Photocopy of the data pages and children pages.)
3. A letter from the school where the child/children is/are studying in Malaysia. (In case of school going children.)
4. A letter explaining why a separate passport is required, if a child is below 16 years of age. This letter must be signed by both parents.
5. A Fee of RM 515 for the issue of a new passport. Applicants are advised to pay passport fees in cash and obtain an official receipt. Applicants are strongly advised not to send their passport fees in cash by post. The High Commission cannot accept responsibility for any loss of cash sent by post. Personal cheques cannot also be accepted for payment of passport fees.

 
Attestation and Authentication of Documents
1. Attestation of Documents as Justice of the Peace / Certification of Documents Fee - RM 175.
2. Swearing or Affirming an Affidavit Fee - RM 175.
3. Administering an Oath or Affirmation Fee - RM 175.

 
Registration of Birth

A birth of a child born in Malaysia may be registered by the parents (Sri Lankan Citizens) at the High Commission in
Malaysia (Kuala Lumpur) on production of the following documents:

1. The child's original Birth Certificate issued by the local authority with 04 photocopies. (English Translations are required for certificates issued in Bahasa Malaysia).
2. Original Marriage Certificate of parents with 04 photocopies (English Translations are required for certificates issued in Bahasa Malaysia).
3. Passports of Parents with 04 Photocopies (Data, Visa, Renewal Pages & Endorsement if any)
4. Original Birth Certificates of parents with 04 Photocopies each.
5. Explanation letter in 02 copies, for not registering the birth of the child within the stipulated time, if the registration is not done within 03 months of the birth.
6.

Letter of no objection for the inclusion of the child in the Father/Mother’s Passport.
(Only for “L” and “M” series Passports)

 
The under mentioned forms should be filled and signed by the father of the child for each child:
1. Form I :Ceylon Citizenship Act No. 18 of 1948 in duplicate (Please get a photocopy from the original.) Download
2. Form II: Declaration of Birth under Section 16 in duplicate. (This form is required when registration is done within 03 months of birth of child.) Download
3. Form III: Declaration of birth under Section 24 in duplicate. (This form is required when registration is done after 03 months of the birth of child.) Download
4. ‘Declaration of Citizenship’ by father of the child in 04 copies. Download
5. Form 'C' - Application for Inclusion of children (only for “L” and “M” series passports).
This should be signed by the spouse or legal guardian of the child/children. Download
6. 03 photographs of the child (size 3.0 cm x 3.5 cm)
7. Form ‘B’151 – Vital Statistics of Birth. Download
 
Fees
1. RM 70 per child for the issuance of the Birth Certificate.
2. RM 33 for registration in accordance with the requirements of Section 5(2) of the Citizenship Act. No. 18 of 1948.
3. RM 20 Endorsement fee for inclusion of the child in Father/Mother’s Passport.
4.

Additional RM 16 per year if there is a delay in registration of child.

 

Fees should be paid personally at the High Commission in cash. Kindly do not transfer money through the bank but pay
to the Cashier at the High Commission and obtain a receipt. Please do not send your fees in cash, by post. This High Commission cannot be held responsible for any cash sent by post. NO CHEQUES ARE ACCEPTED.

Inclusion of Children
In the case of “L” and “M” series passports, names of children under 16 years of age will be endorsed in either of the parent’s passports. Since the inclusion of children’s photographs in “N” series passports has now been automated, the
High Commission is unable to entertain requests for inclusion of child/children’s details into the parent’s passports.
However, the High Commission could issue a Non-Machine Readable Passport or an Emergency Certificate as a
separate travel document until such time the Department of Immigration and Emigration in Colombo processes the application for a new “N” series passport.

Important
It is advisable to have a prior appointment for Birth Registration and for applicant to be present at the High Commission
in Kuala Lumpur. If a child is born to unmarried parents or prior to marriage of parents, an affidavit should be signed by
the father of the child before a Diplomatic Officer (Justice of the Peace) of the High Commission in addition to the above mentioned requirements for which an additional fee of RM 175 is charged.


 
Registration of Marriage

To register a marriage applicants must meet the following conditions:

1. At least one of the parties to the marriage should be a citizen of Sri Lanka.
2. Both parties should not be within the prohibited age of marriage under Section 15 of the General Marriage Ordinance. Therefore,
3. Where a party is below the age of 21, the consent of the father, or in his/her absence the mother, or the guardian (in the absence of both parents) is necessary. {Section 22 (i) of the Marriage Ordinance}.
4. Either party should not already be married.
 

Marriages that fall under the prohibited degrees of relationship:

1. The parties to the marriage should not fall within the prohibited degrees of relationship as set out in section 16 of the General Marriage Ordinance.
 

Procedures for Registration under normal notice of Marriage and special license:

1. If both parties have resided in the country for more than 10 days (Normal Notice)
2. If both parties have resided in the country for more than 4 day (Special License)
 
Documents to be produced
:: Passports of both parties.
:: Original Birth Certificates of both parties and same should be authenticated by the Ministry of Foreign Affairs in Colombo.
:: Passports of witnesses.
:: Authentic documentary evidence will have to be produced to the High Commission that the parties to a marriage have not contracted a marriage earlier and that they are eligible to get married.
:: An affidavit declaring that he/she is not married in Sri Lanka, Malaysia or anywhere else or married and divorced.
:: If one party is a resident of Malaysia, a declaration made in the presence of the relevant authority in Malaysia in regard to his /her civil status and the declaration should have the authentication by the Foreign Ministry of Malaysia.
:: A certificate from the relevant authority of the country of residence stating that he/she has not entered into matrimony in that country. It is necessary to have the document authenticated by the Foreign Ministry of that country or by that country's High Commission /Embassy in Malaysia.
:: Copies of the relevant pages of the passports of the parties to the marriage to indicate the date of arrival in Malaysia.
:: Two witnesses with their proper identification.
Note

English Translations are required for certificates issued in Bahasa Malaysia.

 
Notice of Marriage
Either party who has resided in Malaysia for 4 days immediately prior to the date, on which the application for the notice of marriage is given, can give the notice of marriage.

Once the notice of marriage is accepted, the party can obtain a certificate from the High Commission stating that the notice of Marriage has been handed over to the High Commission and that the official registration will take place after a lapse of 14 clear days from the receipt of the said Notice of Marriage but within 03 months from such date. This certificate is issued without any commitment on the part of the High Commission only for the purpose of going through temple /Church ceremonies. This clause is not applicable when the registration of marriage is done under special license.

The date and time for registration will be notified to the party once the application is processed and accepted by the High Commission.

Registration of Death

A death can be registered at the High Commission provided the following conditions are satisfied:

1. The deceased is a citizen of Sri Lanka, and
2. The death had occurred in Malaysia
 
Documents required
:: Passport of the deceased or if the deceased is a minor the passport of the parent valid at the time of the death is adequate evidence of his citizenship.
:: Death certificate of the deceased along with the Police Report and the Post Mortem Report issued by the competent authorities in Malaysia
:: The duly filled application form (Available at the High Commission).
Note

English Translations are required for certificates issued in Bahasa Malaysia.

 
Procedure for Registration of Death
:: The application for registration of death should be made to the High Commission within three months of the death.
:: When an application is made after 03 months and within one year, an explanation should be made explaining the delay to the High Commission.
:: When an application is made after one year, an explanation addressed to the registrar general should be forwarded, before such a death is registered at the High Commission.

An applicant should be the nearest relative of the deceased or a relative of the deceased who resides in the country where the death has occurred or the main occupant of the house where the death has occurred or the doctor in charge of the hospital/public institution where the death has occurred. If there are no such persons necessary action may be taken to dispose of the deceased by burial, cremation or other means.

Endorsements
A. Endorsement of a Change of Name after Marriage/Alteration to Name/s/ Change of Profession/ Change of Photograph
1. Attach "O" Form, duly completed and signed and forward together with your passport.
2. Attach all documentary evidence causing the change of name, i.e. marriage certificate, passport of husband (original and photocopy), documentary evidence for change of profession etc.
3. FEE: RM 20.
Fees

Fees should be paid personally at the High Commission in cash. Kindly do not transfer money through the bank but pay
to the Cashier at the High Commission and obtain a receipt.

B. All Countries
1. Attach "O" Form, duly completed and signed and forward together with your passport.
